By registering my child or dependent for the Together Initiative Network Day of Caring, I hereby release, indemnify and hold harmless The Factory Ministries (the agency), and the organizers, sponsors, and supervisors of all its activities, from any and all liability in connection with any injury or claim resulting or caused in conjunction with the event, entitled Together Initiative Network Day of Caring on Saturday, April 22, 2017. In addition, I hereby grant The Factory Ministries and Together Initiative Network permission to utilize any comments made by my child or dependent, or photographs or videos taken of my child or dependent, concerning the event. I intend to be bound legally by this release.
